The request form generated during the on-line application process containing your request ID number. | ||

Two forms of photo identification. One of which must be a valid, current, official government ID such as a passport, driver's license, and government issued photo identity card or badge. The second photo ID can be an official company (non-retail club), or institutional, issued photo identity card or badge. | ||
|
Form of payment (Purchase Order Number, Check, Credit Card). | ||
|
Please provide Proof of Organizational Affiliation for Business and Government Certificates. | ||
|
For all certificate types (except Unaffiliated), you must show Proof of
Organizational Affiliation.
If you are using a photo ID badge that shows your company affiliation,
as
one of your two
forms of identification, then this will also work as your Proof of
Organizational Affiliation. If you are not submitting a copy of a
badge that shows your company affiliation, then you will need to
submit a letter on company letterhead, signed by a Duly Authorized
Company Representative, stating that you are an employee of that
organization. (exp. Individual's
Proof of Organizational Affiliation Letter.)
Please note: The Proof of Organizational Affiliation Letter does not take the place of a second photo ID. You must still submit 2 photo IDs if you use the Proof of Organizational Affiliation Letter. |

For Component/Server Certificates, please provide a Component/Server Proof of Organizational Affiliation letter that designates you as the representative for your company for that certificate. | ||

For VPN IPsec Component Certificates, please provide a VPN IPsec Proof of Organizational Affiliation letter that designates you as the representative for your company for that certificate. |

For Code Signing Certificates, please provide a completed Proof of Organizational Affiliation and Authorization for Code Signing document, and Code Signing Attribute Authority (CSAA) Obligation letter along with your certificate application. |

Please note that Unaffiliated Individuals who are requesting Unaffiliated Identity and Encryption Certificates do not have to show Proof of Organizational Affiliation. | ||
|
For Medium Hardware Assurance Certificate, requests must be made in the presence of an ORC RA. Certificates must be requested and issued on a FIPS compliant Cryptographic Token. | ||
